IHCS provides comprehensive, community-based reentry services designed to support individuals returning from incarceration who are at moderate to high risk of recidivism. Through a structured and evidence-informed approach, IHCS delivers integrated care that addresses the underlying drivers of criminal justice involvement, including behavioral health needs, housing instability, unemployment, and lack of access to healthcare.
Services begin prior to release, when possible, and continue through community reintegration for up to 12 months. Each participant receives a validated risk and needs assessment to guide individualized service planning and ensure appropriate service intensity.
IHCS collaborates with correctional partners, healthcare providers, and workforce development agencies to ensure a seamless transition from incarceration to community-based care.
The program is designed to improve outcomes including increased housing stability, improved behavioral health functioning, increased employment, and reduced rates of recidivism.
